I understand and agree that:

• My credit card will be charged once every 30 days for the amount listed above.

• Applicable sales tax and shipping charges (if applicable) may be added to the monthly charge.

• This authorization allows Precision Joint Solution to store my credit card information securely on file and process recurring monthly charges.

• This authorization will remain in effect until canceled in writing by the cardholder.

• Cancellation requests must be received at least 7 days prior to the next billing date to prevent the next scheduled charge.

Barn Group Discount Terms

Participation in the Barn Monthly Supply Program may include discounted pricing based on the number of participating horses within a barn.

If the number of participants within a barn changes and no longer qualifies for the group discount level, pricing may be adjusted accordingly.

Customers will be provided 30 days written notice prior to any pricing adjustments related to barn participation levels.
